Setting a Field Value in a Lookup Field within a List

I am trying to send default values to multiple columns within a list and I cannot.

I am calling the list using newform.aspx?Title=Report%20Error&ReportName=CR%20Detail.

Unfortunately, I cannot do the following iwth this

1. The list does not show these default values if it is a lookup column

2. The list does not show the values if it is not required.

Any insight/help/examples.
