Create Sharepoint Forms

Greetings all,



I am looking for a little guidance if possible.

I am wondering if it is possible to create a form within sharepoint (Blank template) with certain questions and blank boxes for answers etc...



Can this be done via sharepoint or does it need be done by another piece of software that can link into sharepoint?
